  • August 23, 2022: To receive a share of the settlement, you must submit a fully executed Claim Form no later than this date.

  • August 8, 2022: If you want to be excluded from the class, you must submit a written exclusion by mail, fax or electronically on this website on or before this date.

  • August 8, 2022: If you are a member of the class and you do not believe that the proposed settlement is fair, adequate, and reasonable, your written objection must be submitted on or before this date.

  • September 15, 2022: The Court has scheduled a Final Approval Hearing for 9:00 a.m. in the U.S. District Court for the Middle District of Tennessee, 719 Church Street, Nashville, Tennessee 37203.
